PERRL is a quick neurologic check focused on the pupils and how they respond to light. In the field, you’re looking to see that both pupils are equal in size, roughly round in shape, and that both constrict when light is shone on them. This reflex involves the optic nerve (sensory pathway) and the oculomotor nerve (motor pathway) and gives a rapid sense of brain or cranial nerve function. The term does not assess the palate, the retina, or pulses, which are different areas of examination. So the feature being rapid-assessed here is the pupils themselves. If you notice unequal pupils or a lack of light response, that flags potential serious injury or illness requiring further evaluation.
